In Southeast Asia most toilets are right under the shower but they primarily use a bum gun for cleaning up instead of toilet paper. I would be worried about the spray from the shower causing dirty water to splash around when it hits the toilet water. This is a very common layout for bathrooms in some countries. Albeit, the bathrooms are usually much larger, but otherwise it's very common for bathrooms in some countries to consist of a single tiled room with a single drain, a toilet, sink, and shower (plus sometimes a separate tub, too).

If you were so inclined, you could take a shit while showering and clean the poop from your buttcrack. The toilet and shower should be in totally different rooms It is insane that the place we get cleaned is also used as the same place we put out bodily waste. Not only is it kinda gross, it also contaminates everything in the room.
When the toilet was plugged it pushed the clog down the line, downstream of the shower. Now anything that enters the waste piping upstream will come out at the shower. So, from a plumbing standpoint, the question is, do I do/say anything? Is the waste line from the shower really any different from the waste line from the toilet? esp in rural areas don't have plumbing much less modern flush toilets. The shower has a P trap under the drain, the toilet does not so you'd have to add a trap and recenter the new shower drain location. If the shower drain is indeed 4" you won't have to run further back to find a 3" drain and can just reroute it to your toilet location.
